EMPLOYMENT FORMS

All applicants must be agreeable to the required criminal background check required by the State of Florida. In addition to the forms below, you will need to provide a legal photo identification card such as a driver's license. One of the items below is a check list to help you determine if you have completed all documentation.

Another requirement for continued employment is the completion of the 40-Clock-Hour Introductory Child Care Training offered by the State of Florida. You may be hired without the course having been completed with the stipulation that you begin training and complete the training within 1 year of beginning. The course is offered by the state on-line.

All costs associated with meeting the state's requirements to work in a daycare are at the applicant's expense.
